She cited irreconcilable differences in divorce documents filed on Monday (28Sep15).

She lists the date of separation as 3 September (15). The couple wed on New Year's Eve, 2013.

Cuoco, 29, is also requesting her name be restored to Kaley Christine Cuoco.
